Appointment Schedule

Scout Physical day is on Nov 7^th from 4:30 - 8:30 PM at Fort Hill Presbyterian Church.

While this scout physical will fulfil requirements for you to participate in scouting activities, it does not replace the need for an annual medical physical with your provider who has access to your entire medical history. Additionally, note that we will have no special medical equipment or lab capability. If the medical history or exam indicates a need for testing or specialty evaluation, we will not be able to complete your medical forms and you will need to schedule an appointment with your provider to have your scout medical forms completed.

If you are not going to Philmont, follow this link to print out a Scout Physical Medical Forms packet to bring to your appointment. If you are going to philmont, follow this link to print out a High Adventure Medical Forms packet to bring to your appointment. You should also bring one copy of your health insurance card for each person receiving a camp physical to be filed with their paperwork. You must fill out parts A, B1, B2, and the top part of part C prior to being seen by your provider.
