Request Appointments

We have four providers to perform camp physicals. Each appointment slot corresponds to a single provider. If there are multiple people in a family to be seen and you would like them all be seen by the same provider, then only select one slot per time period, and select successive time periods. When this is the case, we will attempt to assign all family members to the same provider.

Please request the appointment slots needed for your family, and then click the "Submit Request" button. Your request will be submitted, and you will be directed to a page which displays your family's appointment schedule as well as a link to view your appointment schedule in the future.
